What is the DA 2125 form for?
The information provided in the report is used to identify the student’s school, to monitor the subjects the student studies, to obtain the student’s response to a specific question, to obtain an Academic Plan, to know the student’s grades and credits, etc. This data is very important for the Army and the military student’s records.
Which documents support the DA form 2125?
There is no need to attach other documents to this report.
When is the DA 2125 due?
The report must be completed by the student and faculty advisor and sent to the Army upon entry into the school at the end of each academic term and when changes to academic program occur.
What information should be provided in the DA form 2125?
The student has to provide the following information:
- Name, grade, SSN, branch
- Current mailing address, home phone, army program
- Name of school, email, type system
- Official title of the expected degree
- Department and major field of study
- Dates of completed and upcoming quarters and semesters
- List of studied subjects and subjects to be studied (title, grade, credit hours)
- Detailed description of all difficulties and troubles that occur during studying
The second page of the form is an academic plan completed by the student. This plan represents an estimate of the number and sequence of courses that are required for satisfactory completion of all academic requirements.
The form must be signed by the student and the faculty advisor as well. The faculty advisor also has to type his name, department and phone number.
What do I do with the form after its completion?
The completed and signed form is sent to the US Army Human Resources Command.
