A CG-2692 Form is a US Department of Homeland Security form, otherwise called the Report of Marine Casualty, Commercial Diving Casualty or OCS-Related Casualty. Who needs the CG-2692 Form?

The casualties that might take place include injury, death, property damage, environmental damage and dangerous conditions.

Is the CG-2692 Form accompanied by any other forms?

Generally, there is no need to accompany the Report of Marine Casualty by any other documents. However, if more than one barge caused or sustained damage in the accident, fill out and attach Form CG-2692A to the primary report.

When is the CG-2692 report form due? 

The Report of Marine Casualty, Commercial Diving Casualty or OCS-Related Casualty must be filled out and submitted within 5 days after the casualty occurred.

A very serious marine casualty means a marine casualty involving the total loss of the ship or a death or severe damage to the environment.
§ 4.03-2 Serious marine incident. (b) A discharge of oil of 10,000 gallons or more into the navigable waters of the United States, as defined in 33 U.S.C. 1321, whether or not resulting from a marine casualty.
