Note the systolic (top) and diastolic (bottom) values in millimeters of mercury (mmHg).

Assess the patient's heart rate: Locate the patient's pulse on the wrist, neck, or groin. Using a stopwatch or watch with a second hand, count the number of beats for 60 seconds. Alternatively, count the beats for 15 seconds and multiply by four to get the heart rate per minute.

Check the patient's respiratory rate: Observe the rise and fall of the patient's chest or abdomen for one minute. Count the number of breaths the patient takes in that time.

Record all vital signs accurately: Ensure that you write down the patient's temperature, blood pressure, heart rate, and respiratory rate in the appropriate section of the medical record.
