These numbers are rising sharply because: (1) The federal program serves a much higher proportion of the working poor than it does a more secure and well-off population of older, self-employed individuals; 2) Medicaid expenditures per beneficiary have grown less than the spending per capita for other similar types of insurance coverage in other developed countries; and (3) Medicare provides a comparable level of assistance at a lower cost. In response, the current system has been designed with a limited scope for innovation that was originally directed at dealing with these two issues. However, there is little evidence that these modifications are reducing spending or improving program performance. We would like to improve program efficiency and help the nation's health care providers deal with the rapidly changing medical environment. Since the fiscal year 1993 Medicare Trustees Report concluded the Medicare program was underfunded, we have concluded that the program's financial position demands changes in the way it operates, particularly the use of beneficiary cost-sharing, improved cost controls, and other efforts to limit the growth of costs that must happen over time. Although we expect these recommendations will take time to be implemented under the current program financing provisions of the Medicare Act, a comprehensive update to the financial picture and some other changes in the medical reimbursement program are essential in order to continue providing quality health care coverage that meets our social and economic priorities. To achieve and maintain this outcome over the next decade, we have undertaken two major initiatives: (1) To study whether the Medicare program's current administrative and financial components can sustain its current cost to the nation; and, (2) To develop and implement a new, more comprehensive approach to providing cost-sharing incentives that are consistent with the spirit of Medicare. We believe that the first initiative is imperative. Improving the quality of health care delivery will require a better understanding of how and when Medicare payment practices encourage efficient payment and achieve value to beneficiaries and providers; improvements in measurement and information are essential to any proposal to deal with this problem. There is a strong body of evidence that improved cost containment is an important component of many quality improvements that are currently being implemented. Yet, despite these efforts, we believe it is time for Medicare to revisit its current approach to the use of cost-sharing. If we are to avoid significant reductions in the health care coverage of future beneficiaries, changes in the use of cost sharing are essential.
