III. Information regarding Student Death and Medical Accidents 1. The Student Health Services (SHS) provides medical and mental health services for students to help them manage their emotional, academic, social and physical issues when there, illness, injury, or death, or for other reasons. SHS will always provide an emergency call within the UW campus for all students in dire need of services. The call should be made by calling (510) 486–4900 or UW-Stout Health Services. 2. When a student's death is a result of an accident or medical event or for unknown reasons, the UW may require notification of parents to the University-wide policy 12-69, Memorials for Deceased Students (see I. Section). 3. Student Health Services will provide notification of the student's death to parents only upon confirmation of death. Parents will also be notified in writing of the student's death. These notifications may be mailed to the deceased student's parent at their mailing address. The student and/or the parent are notified of the notification in his/her UW student record. Parents will also be forwarded a copy of the Notification of Death form. 4. A copy of a Notification of Death form is sent to the UW Student Account Services for the UW student with the last-known UW email address. It will direct the student/parent to the Student Account section online where an account will be set up to handle requests for funeral services. A request for death benefits can also be set up. If applicable, the student/parent is notified of any other changes to his/her records by contacting the Student Account Services at 925.515.3151. A statement of nonrefundable/non-refundable funeral care expenses can also be included to the Student Account section. Parents will receive a Student Account form that includes information about funeral services. 5. The funeral home must have current contact information for the deceased, the parents with the last known UW email address (if applicable), and/or a UW-wide mailing address. If the funeral home does not currently have these, the funeral home should have a forwarding address or an email address on file for the UW Student Account Services. The funeral home must confirm that there are no legal documents attached to the death. 6. The UW-Stout Memorial Services Office will process any request for a deceased UW student's memorial service.
