These new forms will include the following: (1) an affidavit providing that the applicant has adequate means of support, or that he intends to take employment in the county of residence; (2) verification of applicant's Social Security number; (3) information, in the form of a birth certificate, to determine eligibility for food stamps; (4) information, in the form of an IRS Form W-9, identifying the applicant's income; (5) payment of 5 per person or 25 per family for the Food Stamp. The food stamp card must be in the name of the applicant or a designated individual. If the applicant is not the biological parent, it must be issued to a dependent of the applicant. The Department expects to spend around 35 million to issue the forms by the end of 2004. The Department anticipates additional expenses for training county welfare officers and other agency personnel. NOTE: FOOD STAMP INFORMATION: A county welfare director may approve or deny a food stamp application on an individual basis. To view an applicant's application, you will print the original paper Food Stamp Application Form and fill out an application for Food Stamp Services with supporting documentation. You have the option of faxing your application to. To receive an application via the Internet, you will print the application form, complete it electronically, and save it to your computer. Food Stamp Fees Food Stamp Form — 3.00 Application fee (not per person) includes all required information (name, date of birth, Social Security number) and the 5 non-refundable food stamp application fee (applies only to the first 2,500 of eligible food for each of the next 3 consecutive taxable months). The Department has provided two different methods for an applicant to pay the application or application fee: by credit card or by money order. The credit card or money order may be paid by credit card and may be used on a regular basis for the entire three months the card remains in an applicant's account.
