Intraosseous anesthesia (IA) allows direct placement of the anesthetic solution in the cancellous bone adjacent to the tooth programmed for anesthesia. Since the anesthetic solution is targeted directly to the tooth requiring treatment, the surrounding soft tissues are usually not affected (1).
The most commonly reported complications of IO access are infection at the injection site, which may result in severe osteomyelitis, damage to the growth plate, and embolism, 2 which have been reported in adults.
Intraosseous anesthesia (IOA), an important technique in dental anesthesia, delivers anesthetic directly into the bone, enhancing patient comfort and minimizing side effects. Despite advantages, several disadvantages are known such as injection discomfort, root damage, mechanical obstructions and needle fractures.
The intraosseous injection allows placement of a local anesthetic solution directly into the cancellous bone adjacent to the tooth to be anesthetized.
Known rare complications of IO access include extravasation, soft-tissue necrosis, bone fractures or injury to growth plates, infiltration of medications, infection, subcutaneous abscess, osteomyelitis, and embolic complications ( emboli). Compartment syndrome has also been reported.
Emergency prehospital intraosseous cannulation may theoretically lead to an increased risk of long-term complications such as osteomyelitis, osteonecrosis, or compartment syndrome.
In conjunction with a tourniquet, IO access enables rapid fluid replacement. Another advantage of intraosseous access is the ability to perform basic tests for blood gases and to determine blood type. However, their main disadvantage is the high costs of the intraosseous needles and sets.
The intraosseous (IO) anesthetic technique requires mechanical perforation of the thick cortical plate between the roots of the teeth to permit deposition of the local anesthetic into the medullary bone sur- rounding the tooth.
