(2) The application may be filed on behalf of a group of separate groundwater discharges of sewage to the same groundwater source pursuant to the provisions of sections 4-1.1, 7-3.2, 7-8.1, and 7-10.4. (3) The total volume of wastewater discharged from the permitted area may not exceed 10,000 gallons per day. The minimum volume to be discharged shall be equal to the volume of the highest discharges to the surface. (4) The fee for processing the permit application must be paid by the party filing the application and the cost of collection. (5) The holder of this permit shall not discharge to the groundwater above a depth of 10 feet from the surface unless such discharge is authorized in writing by the agency head or agency designee, or by a permit application filed in the office of the agency head or designee pursuant to subsection (1). (6) A permit shall terminate upon termination or expiration date or upon the earliest of the following: (a) The discharges have caused or will cause an adverse effect to the environment or the quality of the groundwater in a manner which would require an emergency permit under this chapter; (b) The permit applicant for a permit to the surface has failed to comply with the provisions regarding adverse effects, if any, and has failed to request and be granted an exception pursuant to section 7-3.2(c), 7-15.4(c), 7-15.
