This form, Postage Statement — Nonprofit Standard Mail Easy-Nonautomation Letters of Flats, ?is designed to be used by the nonprofit mailers and should be completed by them for nonautomation letters or flats at Nonprofit Standard Mail prices that are sent via the United States Postal Service (USPS).
What is PS Form 3602-NZ for?
The mailer’s signature certifies that the income derived from the sale of any products or services advertised in the mailing is not subject to the Unrelated Business Income Tax, and any products and services advertised are substantially related to nonprofit organizations.
Is PS Form 3602-NZ accompanied by other forms?
There is no need to accompany this Postage Statement with any other forms. However, you have to attach a check for a fee payment.
How do I fill out PS Form 3602-NZ?
In the Mailer block, the following must be indicated:

Total adjusted postage affixed and permit imprint must be indicated to complete the form correctly.
After this, you should read and sign the Certification section, including your telephone number.

Where do I send PS Form 3602-NZ?
Once completed and signed, the 2012 3602-NZ form should be directed to the Postal Office and a copy should be kept for the personal records.
