How to fill out blood pressure assessment:

01
Start by gathering the necessary equipment, including a blood pressure monitor, cuff, and stethoscope or digital monitor.
02
Choose a quiet and comfortable location for the assessment, ensuring the individual is seated and relaxed.
03
Wrap the cuff around the upper arm, just above the elbow, with the bottom edge placed approximately 1 inch above the bend of the arm.
04
Position the stethoscope or digital monitor over the brachial artery, which is located on the inner side of the elbow.
05
Inflate the cuff until it is snug but not overly tight, making sure the person's arm is relaxed.
06
Slowly release the pressure in the cuff while listening for the person's heartbeat with the stethoscope or observing the digital monitor.
07
Note the reading when the first clear thumping sound (systolic pressure) is heard or displayed on the digital monitor.
08
Continue deflating the cuff until the sound disappears (diastolic pressure) and record the reading.
09
Repeat the process at least twice to ensure accuracy and consistency.
10
After completing the blood pressure assessment, document the readings along with the date, time, and any additional notes.
