The operator of an aircraft must immediately, and by the most expeditious means available, notify the nearest National Transportation Safety Board (NTSB) Field Office when: An aircraft accident or any of the following listed incidents occur: Flight control system malfunction or failure.
Investigation of accidents consists of three phases (see Figure 1.1): a) collection of data, b) analysis of data, and c) presentation of findings. The initial phase of the investigation process should focus on defining and obtaining data relevant to the accident.
The operator shall file a report within 10 days of an accident. An accident is defined as “an occurrence… in which any person suffers death or serious injury, or in which the aircraft receives substantial damage.” Any of the other events listed in 830.5 are reported “only if requested by an NTSB representative”.
