How to fill out roe51:

Report any other payments made or to be made to the employee during the period of interruption, such as severance pay, retiring allowances, or pay in lieu of notice.

Provide any additional information or comments that may be relevant to the ROE or the employee's circumstances.

Review the completed form for accuracy and make any necessary corrections or adjustments.

Sign and date the ROE, and provide your official business contact information.

Submit the completed ROE to Service Canada within five days of the employee's last day of work, either electronically through the ROE Web service or by mail.
