Field Trip Permission Form 2016 2017 This permission slip will be used for day field trips for the 2016-17 school year. Only one form per family needs to be completed. Please be sure to list the name and grade of each of your children who attend Westminster. Once this form has been completed it will not be necessary to turn in another form with the exception of overnight trips. This permission slip must be signed and returned before a student is permitted to participate in a field trip*.
