Veterans who want to apply for medical, educational or employment benefits at their regional VA office should sign the form 2122 in order “to hire” a service officer to represent him and help with the claim.
What is form 2122 for?
It is a legally binding document that authorizes an agency or a person to be the veteran’s representative in a chosen case (or several cases). Once it’s been signed, the authorized party obtains access to the claimant’s VA file and medical records unless the restricted documents that they discuss are included in this form. The authorized agency needs this authorization in order to apply for veteran benefits, or represent the applicant in court or at medical facilities.
Is fillable form VA 21 22 accompanied by other forms?

When is form 2122 due?
The form is between a claimant and an agent who decide on the due date. If the claimant is not satisfied with the agent’s job, he needs to terminate this form and sign a new one for a new appointment.
How do I fill out a VA form 2122?
Write the name of the veteran and his VA file number. Add the name of the organization chosen as the representative. There is a list of the organizations approved by the Department of Veteran Affairs on the second page of the PDF form VA-21 22. Write the name of the agent who will be dealing with your case on behalf of the organization. If you’re not a veteran, give your social security number, your insurance number and determine your relationship with the veteran. Check the box to verify your authorization and limit the consent to disclose medical records if you need to. Don’t write anything in the bottom of the page — it’s for the VA office.
