Who needs a cover sheet?

Everybody who wants to protect his intellectual property and receive a patent for invention must file this cover sheet and a provisional application for patent with United States Patent and Trademark Office. It can be used by an individual or by a group of inventors to describe one invention they would like to register.

What is this cover sheet for?

This cover sheet represents the main page of a provisional application for a patent. It must contain general information about the invention, its author(s), their contacts and mailing addresses.  The rest of the application will provide more details about an invention, including drawings and writings with precise descriptions.

Is it accompanied by other forms?

Yes, it is. Inventors must file this cover sheet alongside provisional application papers (writings and drawings) and a filing fee. Fees are subject to change annually. See current fees at www.uspto.gov. Payment by check or money order must be addressed to "Director of the U.S. Patent and Trademark Office."

When is this form due?

Once the inventor made a public disclosure of his discovery, he has 12 months to apply for a patent. Instructions to this form define public disclosure as public use, publication or offer for sale.

How do I fill out a provisional application for patent cover sheet?

In the cover sheet applicants must identify:

  • That this application is a provisional application for patent

  • The names of the inventors

  • Their places of residence

  • The title of the invention

  • Name and registration number of the attorney or agent and docket number

  • Correspondence address

  • Any U.S. government agency that has a property interest in the application

Hey I'm Bob from 12th floor solutions, and I'm really excited to show you some of the cool stuff we've been working on in flow Microsoft flow has been around for about a year now and really starting to gain some good traction, and we're going to show you how to set one up and to end this particular flow starts in SharePoint where a user can create a vacation request once approved it goes into a calendar that everyone can see so let's get started okay before we start creating the list and the flow I'm going to show you conceptually what the flow is doing, so first an item is going to be created by a user and that's going to act as the request submission, and we'll set up a vacation request list to do this once that happens the flow will kick off, and it will send an approval using the full approval connector when the user who's assigned the approval approves it is will create an item in the vacation calendar where everyone will be able to see it we're going to do one extra little step which is we're just going to set the original item in the vacation request list to approved and that's going to essentially firm up the item so that the original user can see that it's approved and can't change it okay the first thing we're going to do is we're going to set up our lists so the first list we're going to create is a custom list which is the vacation time request and that's where the users are going to input their time to request their vacation, so it's just a standard custom list the other list we're going to create is the calendar and that's going to be the public calendar where all the users can see approved vacation requests, so it's just a calendar click create, and now you have your two lists created we're going to go back into the vacation time request, and we're going to create two custom columns and those columns are going to be the start and end date of the request okay, so we'll enter the start date and this will be a date and time field we're going to make it date and time so that if you have your vacation halfway through the day that's totally fine, and we're going to do the same for the, so it's going to be a date and time field, and you can enter the date which the vacation ends okay once that's all created we're going to enable content approval on this list and the reason is just to make it more official so that once it gets approved the item is approved and users can see it within the vacation or time requests if you need it for reporting or any other purpose, but this isn't a required step if you wanted to get started okay now we are going to once your lists are created you can see them here we're going to start the flow we're going to find the flow application, so I'm just going to type flow click on it that's going to load up the dashboard for creating flows, so I'm going to create a new one from a template, and I'm going to do it when an item gets created in SharePoint, so it's going to create an empty dashboard where I can set up...
