What is Form UI-28?
Form UI-28 is called the Employer’s Claim for Adjustment or Refund. This form must be filed by the employer.
What is Form UI-28 for?
The form is created for claiming any refund or adjustment. All these claims may be subject to audit and field investigation at the Director’s discretion. If the claims involve contributions that were paid on overstated salaries, they may influence the contribution rate of the employer for any year.
When is Form UI-28 Due?
This form must be filed by the employer not later than 3 years after the date when the contribution, penalty or interest were erroneously disbursed. Generally, the claim must be completed every calendar year.
Is Form UI-28 Accompanied by Other Documents?
Other forms or documents are not required to be attached. However, if more space is needed for providing all information, it is possible to attach more additional sheets.

The following information must be indicated in the form:
