A photocopy of the written or printed petition, accompanied by a certified copy of the affidavit executed under penalty of perjury, and accompanied by the 80.00 fee set by the Secretary of the State, shall be sufficient for all purposes to initiate the proper application with county clerks or elections officials for issuance of a new petition. Any person who is the subject of a previous court injunction, or is the subject of a pending appeal, or is the subject of a criminal conviction involving domestic abuse or stalking, who has not filed a certified copy of the petition, or who desires to be re-entered shall execute and file a new petition with the clerk of the county in which he or she resides within seven days of the date of the final judgment. I understand, under penalties of perjury, that any person who is a candidate for nomination or election or who is the subject of a nomination or election petition, as specified in this petition, may be prosecuted under federal law for making false statements on a return to the United States Department of the Treasury or making other falsified or fraudulent statements, in order to obtain nomination or election.
