Hello my name is William from Pomona courthouse assuming that John Doe is litigant let me guide you through the FL — 20foraum I want to bring to attention that I will approach this forum for general cases if you believe your case to be a typical please visit one of the self-help centers for assistance click here for information about the self-help centers first fill out the captions if you do not know how to fill out the captions click here in order to save time I already pre-filled out my captions so John Doe's address and the information of the court you're filing a case in now for this box I want to point out that it is important that you write your name and the respondents name the same way for all future court documents this is because the court wants consistency so in my examples I will always write the petitioners name as John Doe and the respondents name as Jane D Doe not Jane Doe not Jane Diana doe but Jane Doe for the boxes below check on all three boxes child support visitation and child custody even if you are not interested in these issues all three boxes must be checked off because the court will address them and case number leave a blank and let's scroll down for number one if you are the mother check off 1a if you are the father check off 1b for the sake of our example I will check off B for number two write the name or names of children involved in this case their date of birth age and male or female so for John Doe I'm going to say that his first child's name is it's called Johnny doe and date of birth it was born July 1st 2009 which in making five years old, and he's a male and for his second child let's say that her name is Jana Elsa dough and birthday her birthday is for two what one should make her three years old and effort female now every time you write your children's names for future court documents you will also write them the same way as you have them so here so every time I write Johnny's name for future court documents I will write his name has Johnny doe and every time I write Jamie's name for future court documents I will write Jamie's name as Janie Elsa doe down here checkoff box 3a because the respondent lives in California and down here checkoff box for a because the child lives in this county check 5a if the respondent is the child's mother or check 5b if the respondent is the child's father for the sake of our example I will check off box 5a because Jane D though is the mother of John those children now if the respondent has failed to support the child check box D if public assistance is being provided to the child check box F for the sake of our example we will leave these boxes blank and let's move on to the second page make sure that above the captions are filled out petitioner is your name responding his name of the other party a case number leave a blank for number 7 parent-child relationship X both a and B for number eight this is where I gave you my custody speech there are two different types of...
