Who needs a form VS-170?
Form VS-170 is supposed to help anyone, who needs corrections in a birth certificate. Any US citizen can use it also to add information, if one of the fields was left blank. This rule applies to all cases except for adding Father’s name. Applicants have to contact Vital Statistics on this matter.
What is form VS-170 for?
Form VS-170, or Application to Amend Certificate of Birth, was designed in the State of Texas and last revised in 2005. It is mostly used to add information to the item left blank in a certificate of birth, or to correct a mistake in spelling, if names have the same sound, or for any other changes in this document.
Is it accompanied by other forms?
Depending on the type of information to be changed, there are different documents to provide with this form. It could be hospital record of birth, baptismal certificate, school record, birth certificate of registrant’s older brother or sister, insurance policy application, armed forces discharge papers, social security application, record of immigration and naturalization service, passport, marriage record of parents, birth or death certificate of registrant’s parents, divorce decree, judicial actions. Applicants don’t have to attach all these documents, only the ones that are related to the requested changes.

Simply fill out the first table. Furnish it with the information from the original certificate. Use Part II to explain what items need to be corrected. Arrange information into three columns: Item, Entry on Original Certificate and Correct Information. Have an older relative to fill out and sign an Affidavit on this form.
