Excellent website with great features I receive documents from my client's customers that need to be filled out and signed by my client. These are handled by PDFFILLER quickly and efficiently. It's very easy to fill in PDF files and it's just as simple to add and insert signatures. Sometimes movements within the website are a bit lengthy. After downloading a completed document, returning to the exact page would be nice.

Please submit evidence documenting your correct identifying information (such as a certified marriage certificate or birth certificate) and your current U.S. passport book and/or passport card, along with this completed form to the address listed on page 2 of the instructions.
Can I print it in just black ink? Thank you. Printing the DS-11 in black and white is just fine.
Renewal of regular U.S. passport for an adult (DS-82 form)
Make sure you prepare the right form, e.g. for a minor child prepare the DS-11, for regular adult passport renewals you need the DS-82, etc.
You can fill out the form using the online form filler of the Department of State website (https://pptform.state.gov/) or print it out and fill it out by hand using a black pen.
To correct a data or printing error: Complete Form DS-5504. Submit form by mail with your passport, one color photo, and evidence of the error (e.g. your U.S. birth certificate showing the correct spelling of your last name). You will not have to submit any fees.
