What is a program evaluation questionnaire?

A program evaluation questionnaire is a tool used to gather feedback and assess the effectiveness of a program. It typically consists of a series of questions designed to measure participants' satisfaction, experiences, and outcomes related to the program. This information is valuable for program managers and stakeholders as it helps inform decision-making, identify areas for improvement, and determine the overall success of the program.

What are the types of program evaluation questionnaires?

Program evaluation questionnaires can vary in their focus and design, depending on the specific needs and goals of the program. Here are some common types of program evaluation questionnaires:

Satisfaction questionnaires: These questionnaires assess participants' satisfaction with various aspects of the program, such as content, delivery, and overall experience.
Outcome questionnaires: These questionnaires measure the outcomes or impact of the program on participants. They may ask about changes in knowledge, skills, attitudes, behavior, or other relevant outcomes.
Needs assessment questionnaires: These questionnaires aim to identify the needs and preferences of the target population or community before developing or implementing a program.
Process evaluation questionnaires: These questionnaires focus on evaluating the processes and activities involved in implementing the program, such as the quality of program delivery, adherence to protocols, and participant engagement.

How to complete a program evaluation questionnaire?

Completing a program evaluation questionnaire is a simple and straightforward process. Here are some steps to follow:

01
Read the questionnaire carefully: Take the time to understand each question and its instructions before providing your response.
02
Provide honest feedback: Share your genuine thoughts, experiences, and opinions. Your feedback will help improve the program.
03
