The complaint has a First Count, Second Count, Third Count and Fourth Count. The form did not allow editing of the formatting to insert space for identifying the "Count" being addressed. Otherwise, the form was fine.

Save PowerPoint presentations as PDF filesSelect File > Export.Click Create PDF/XPS Document, then click Create PDF/XPS.In the Publish as PDF or XPS dialog box, choose a location to save the file to. Optionally, if you want to change what the final PDF file is like, do any of the following: Click Publish.
Open the PDF and choose Tools > Protect > Encrypt > Encrypt with Password. If you receive a prompt, click Yes to change the security. Select Require a Password to Open the Document, then type the password in the corresponding field.
