OK. some limitations, like I need conditional logic, so if someone checks the Yes box for a question, then require them to fill out other fields. Need this conditional logic functionality, critical for my application. Else not sure PDFFiller will be useful to us.

i have had a great experience so far it is much easier to file medical claim forms than having to buy that specific software i like how easy it is to import documents. I also like the ease of sharing, printing and storing documents Really the only thing i do not particularly care for is the difficulty adding the individual box for editable fields

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
The tumbling E chart tests the visual acuity of young children and others who can't read letters aloud. 20/20 vision is considered “normal” vision, meaning you can read at 20 feet a letter that most human beings should be able to read at 20 feet.
The top number in the fraction is always 20it's the number of feet you are from the eye chart. The bottom number indicates the distance at which a person with normal eyesight can read the same line.
If this is the smallest line a person can read, the person's acuity is “6/12” (“20/40”), meaning that this person needs to approach to a distance of 6 meters (20 ft) to read letters that a person with normal acuity could read at 12 meters (39 ft).
All About the Snellen Eye Chart The number of letters on each row increases moving from top to bottom. The size of the letters progressively decreases, allowing for more letters on each subsequent line. The traditional Snellen eye chart only makes use of the following letters: C, D, E, F, L, O, P, T, and Z.
A standard Snellen vision testing chart from the 1950s. The most commonly used eye chart is known as the Snellen chart. It usually shows 11 rows of capital letters. The first line has one very large letter.
Using a Snellen Eye Chart The chart is usually read while standing at a distance of 20 feet.
In the most familiar acuity test, a Snellen chart is placed at a standard distance: 6 meters. At this distance, the symbols on the line representing “normal” acuity subtend an angle of five minutes of arc, and the thickness of the lines and of the spaces between the lines subtends one minute of arc.
20/20 vision is considered “normal” vision, meaning you can read at 20 feet a letter that most human beings should be able to read at 20 feet.
The visual acuity test is used to determine the smallest letters you can read on a standardized chart (Snellen chart) or a card held 20 feet (6 meters) away. Special charts are used when testing at distances shorter than 20 feet (6 meters).
The eye exam charts are not the same for all purposes. You are likely referring to the Snellen visual acuity chart. As above, the chart can be validated for illiterate people or pretenders. Obviously, there are lots of different eye charts for non-Latin-alphabet readers.
