No r al complaints. Did find out that the forms in your database that I was interested had been superceeded by new forms back in Jan of 2016 a year ago. This meant I had to downloaded ad new forms t fill out. You could strive to be more up to date.

Because the text where the footnote occurs is so far down on the page that if Word made room at the bottom for the footnote text, it would force the paragraph with the footnote to the next page. To fix this, you have to move your text around so the footnote occurs further up on the page, or on the next page.
Keeping the start of a footnote on the same page as its reference. If you have a footnote reference in the very last line of a page, there will not be room for an extra footnote, and both reference and note will have to be carried to the next page.
Right-click on the paragraph at the top of the second page and select Paragraph. On the Line and Page Breaks tab of the Paragraph dialog, make sure that “Page breaks before” is not checked. If all three can't fit on the page with the heading, then they will jump to the next page.

The most commonly recommended abbreviation for continued is cont. Cont'd is also a correct way to make continued shorter. It's a contraction, rather than an abbreviation. When writing continued on next page you should consider spelling out the full phrase instead of using a shortened form.
