The initial is a marking on the end of the page. For physical documentation, it is done by ink. Much like a signature, the initial is not something generic but rather, exclusive to the individual using it. It's a small personalisation to acknowledge you have read the page and agreed to the terms on it.
The first letter of your name is your initial. The first thing you say to someone is your initial greeting. Initial is something that occurs first or at the beginning. If someone asks you to initial a form, they're asking you to sign by writing your initials on it.
It means that every page of the document must have your full initials at the bottom right or left hand corner. If the document lists you as Tom Jones, then put TJ. If the document lists you as Thomas John Jones then put TJJ at the bottom of every page, even the page where your signature appears.
Traditionally, the first letters of their first, last and middle name are used, in that order. For couples, if they share their last name, the last name remains in the middle with the initials of their first names on the left and right side.
For an individual, the order of initials is the first name, last name, and middle name. The last name initial (center) is larger than those on either side.
Generally, you use the first letter of your first and the first letter of your last name as your initials, but you can also include the first letter of your middle name or maiden name, or more than one letter from one of the names (e.g. someone with the last name DiAmico using both D and A).
Initials are the capital letters which begin each word of a name.
