In the Forms menu, select Add or Edit Fields For the field you want to edit, access the context menu and select the Properties dialog. In the General tab of the Properties dialog, type a description for the form field in the Tooltip field. Repeat for all form fields.
Place the field at your desired location. Double-click the field to open the properties of the field. In the Name box, type the field name and specify if you want the field to be required.
Select Start. Choose the Add a Signature block icon from the tools ribbon. Move your cursor to where you want to place the block and click. Click outside the new signature block, then save and close the document.

To find the name of your PDF field, first open your PDF template in Adobe Acrobat and enter Prepare Form mode. Next, double click on the field (or right click and select Properties) to open the field properties window. You will find the Name setting under the General tab within the properties window.
Open the document in Acrobat and select Edit > Prepare a form > Create form to convert your document to a PDF form. Choose a form field from the left pane. Once you hover the cursor over the document, a preview of the field field is displayed.
