I love that I am able to correct and hold prior to sending out my file for signature. I wish I could however have a authorization statement from the Calyx Point file to use more of there forms.

I used the service like the service, but until I got to print a copy of the SR-1 form, I thought I was using the government's free website for submitting an SR-1 form. I want to cancel it because I will not need to use it again. So, please cancel my account and service. Please also send an email to me confirming that you have cancelled the service. Thank you.

My first time

A sidebar will appear on the right side of the screen with various adjustment options.

Use the 'Move' option to reposition the chart or attestation within the document. Simply click and drag the chart or attestation to the desired location.

If you need to resize the chart or attestation, use the 'Resize' option. Click and drag the corners or edges of the chart or attestation to adjust its size.

To rotate the chart or attestation, use the 'Rotate' option. Click on the rotation handle located above the chart or attestation and drag it to the desired angle.

Can one physician sign for another?
A signature log should include the physician's printed name, full signature, and initials that appear on the document.
