It took me awhile to understand how to work this software but once I got my file downloaded it was great and saved me a lot of time not having to retype each form

How do I amend my birth certificate?
Contact the office of the court that handles name changes local to you.
On your court date, appear in front of the judge and let them know why you wish to change your name.
Contact the vital records' office responsible for issuing your original birth certificate.
How much does it cost to amend a birth certificate?
