4 starts because of great chat support but some features I would change. Like when filling out fillable form, I don't like how the instruction window follows you through the entire document. Most other online forms don't have this because they assume the form is self-explanatory. The feature is cumbersome, like dragging around dead weight. Anyways, that is my input.

Google Forms does not allow respondents to upload files. There are some ways around this though: Have them upload the file directly to your Drive account.
You can upload an image, take a snapshot, direct the form to a URL, browse your Albums or your Drive, or do an online search for an image. Once you upload or select your image, it will be incorporated right into the question.
To allow image (or file) uploads in a Google Form, select the File Upload question type from the dropdown menu on a question.
Install the Google Add-on. Install Document Studio from the Google store. ... Create a Template. Build your own template with Google Docs, Google Sheets or Google Slide and use {{placeholders}} for merge fields that vary in each document. Configure Data Merge.
Click Insert, point to Chart, and then click on From Sheets. From the list of available spreadsheets, select the one you want to use and then click Select. The next window lists any charts on that sheet that are available for import. Click one and then click Import.
Then click on Settings > Manage apps. Click on the link at the top of that window for “Connect more apps.” When the list of apps appears, type “Merge” in the search field. Then look for “Merge Google Documents, Spreadsheets” and select to connect it to Drive.
In order to merge cells in Docs, simply highlight the cells to be merged, then right-click and choose the Merge cells option from the pop-up menu.
