The forms are good but they would be much better if the user could "snap to" the proper location when typing into a box. Without a "snap to" type guide it is difficult to place the cursor in the correct location within a box and especially difficult to get columns to line up.

Dear (Name of landlord or manager), This letter constitutes my written (number of days' notice that you need to give based on your lease agreement) day notice that I will be moving out of my apartment on (date), the end of my current lease. I am leaving because (new job, rent increase, etc.)
Dear [Landlord's name/Property Manager's name/Apartment Manager's name], As per my rental agreement, I am providing this letter as a [##]-day notice that I will be moving out of my rental unit on [date], ending my lease that began on [date]. This letter shall serve as my written notice of intent to vacate the premises.
Step 1: Start by reading your rental agreement. This doesn't mean just skimming through it to get to the good parts. Step 2: Put your move out notice in writing. Step 3: Determine the best way to deliver your notice to vacate to your landlord. Step 4: Keep a record.
Check your lease to see if you need to send a formal letter. Keep your written notice simple and polite. Don't forget to include the date on your letter. Include your forwarding address.
Termination Clause in the Lease For example, if the lease requires you to give 30 days' notice and pay a fee of one month's rent, then give the landlord 30 days' written notice, pay the fee, and make sure you're out of the apartment within the 30 days.
Put the Roommate on Notice Give a deadline by which the roommate (and the roommate's personal property) must be out of the rental. Even though the roommate isn't an official tenant, you should give at least the same amount of notice required to end a month-to-month tenancy. In most states, the notice period is 30 days.
Include your name and the rental address, and date the letter. Don't date it and hold onto it. Date it for the day you are giving it to the landlord to start the 60 days. Address the letter to the landlord with a subject line of “60-Day Notice to Vacate.”
Be in writing. Say the full name of the tenant or tenants. Have the address of the rental property. And. Say that the month-to-month tenancy will end in 30 days if the landlord is giving a 30-day notice or in 60 days if he or she is giving a 60-day notice.
