What information is required to be included in a breach notification?
The HIPAA Breach Notification Rule requires covered entities to notify affected individuals. HHS. And, in some cases, the media of a breach of unsecured PHI. Generally, a breach is an impermissible use or disclosure under the Privacy Rule that compromises the security or privacy of PHI.
How soon must a HIPAA breach be reported?

What is a reportable breach under HIPAA?
The unauthorized acquisition, access, use, or disclosure of unsecured PHI in violation of the HIPAA privacy rule is presumed to be a reportable breach unless the covered entity or business associate determines that there is a low probability that the data has been compromised or the action fits within an exception.
