They have a form for everything, and if they don't, they will find one for you!! This program is easy to use and I love how there is a record kept of everything I send.
I don't know how to re-use a form over and over without having to delete whats on it , so I can use it again. I wish there was an option to use a new one!!

Do I need to update my DOT number?
Online: You can update your US DOT number company information on the Licensing and Insurance (L&I) website by filing an MCS-150 (Motor Carrier Identification Report). To update online you will need a Personal Identification Number (PIN). You can apply for your PIN online.
How often do I need to update my DOT number?
Biennial Updates Federal Motor Carrier Safety Administration (FMCSA) requires all entities under its jurisdiction to update their information every two years.
How often do I have to update my MCS 150?
What Is the Biennial Update? The biennial update is a requirement set by the FMCSA to update your MCS-150 Form (also called the Motor Carrier Identification Report) every two years, keep in mind some states actually require you to update it every year as part of renewing your truck/fleet registration.
How do I file a biennial update?
Go to www.fmcsa.dot.gov/BiennialUpdate, sign in using the PIN provided in your warning notice from FMCSA, and complete the biennial update process. You may also complete the biennial update process by mail or by fax.

You must complete your biennial update every two years. The month in which your update is due will depend on the last digit of your US DOT number. If your US DOT number ends in 1, for example, your update is due in January. US DOT numbers ending in 2 must be updated in February, and so on.
