Filing a claim is easy with AKC Pet Insurance! Visit any licensed vet or clinic in the US or Canada, pay your bill, and get an itemized receipt. Submit your claim in the customer portal or by email, fax, or mail. Whatever's easiest for you! Eligible expenses will be repaid by check or direct deposit (your choice).
It's easy to submit a claim! Fax, mail or email your form with invoice(s) within 270 days of treatment. If you use email, just scan and attach the form and invoice(s). Include a copy of your pet's medical records to help expedite processing. Please use only one claim form per pet for each accident or illness.
Make a paper claim Fill in Pet Insurance Claim Form with your pet's personal information and sign. Take the Claim Form to your vet and ask your vet to complete in full Part 2 and sign the form. Attach the original detailed itemised invoices and payment receipts to the completed claim form.
