What is the purpose of the Sample Family Child Care Contract?
The parents of a child and a daycare provider use this sample contract to create a legally binding contract that states terms and conditions of the daycare.
What information should be provided?
The parties to the contract fill in their names, names and dates of birth of the children, amount to be paid for the services, daycare schedule including the arrival time and pick up time, late pick up fee, late payment fee, overtime rates, payments during holidays or vacations, additional charges for certain supplies, special trips, damaged property, etc.
If the family uses the state subsidy program (Care 4 Kids), the payment terms should be set out in section Care 4 Kids Payments.
The parties sign and date the contract. If the parent or legal guardian is under 18, the contract must be signed by a co-signer who will act as guarantor and will be bound by financial terms of the contract.
