The application is deemed to be approved upon receipt of (a) a copy of the specific Coverage Endorsement in response to verification from the insurer of the specific coverage required by the specific Coverall er, (b) one hundred fifty dollars (150.00) at a discounted rate for two (2) years from the effective date of the insurance, (c) any other appropriate proofs furnished by any other insurance company, and (d) the application fees. In case of a rejection the insurance applicant will be required to return the application fees. (Approved by ORD. No. 3-04, 2008; Amended by ORD. No. 10-12, 2010; ORD. No. 1-15, 2015; ORD.
