If the professor's approval is denied or the request indicates otherwise, the professor must inform the CR personnel of the reason for the denial or denial will not be honored. Faculty who request to reserve space for more than one student or group must submit a separate form. The name of the CRN must also be entered in the space reservation form. 1. Schedule A, Activity #: Date / Initials: University 3 Hours 1-15. A. Registration form will be required. No new registrations will be accepted. B. A form will be required for each student being registered (e.g. name, phone #, e-mail). When all required information is entered, a space request will be accepted. If any problem is encountered, please contact the CR staff with “Office of Campus Reservation” in the subject line of the email. After the registration form will be made available for review the professor of the activity must send the form along with a check for 35 in cash or credit card made payable to the UBC Flanagan Faculty of Teaching and Research. B. A copy of the current schedule must be attached to the form, along with a separate copy. C. When completing the form, the Faculty of Teaching and Research (FOR) staff must mark the “Student or Employee Reservation Form,” to indicate if the “Student” or “Employee” is occupying the room. It will be acknowledged if no accommodation is available for the individual and provided that all the requirements listed below are complied with. 1) An application form should be completed, a copy of the approved CRN and the name, credit card or other form of payment must be provided. 2) Students must be enrolled in the same UBC courses that they wish to reserve space for by the date of reservation. 3) The space may be reserved for a minimum of three hours in any single unit. 4) If space is reserved for more than three hours, a separate form for each hour must be completed. 5) A check for 35 in cash or credit card made payable to the UBC Flanagan Faculty of Teaching and Research will have to be made for each individual. 6) The CRN of the faculty member will be recorded on the form. The faculty member will be able to identify who is registering. 7) Each faculty member will be responsible for making the space reservation. 8) The University reserves the right to refuse any registration.
