In this workout you will complete 5 minutes of cardio (5 exercises for 1 minute each), 4 minutes of lower body (4 exercises for 1 minute each), 3 minutes of upper body (3 exercises for 1 minute each), 2 minutes of core (2 exercises for 1 minute each), and 1 minute of cardio (1 exercise for 1 minute).
I just use a physical journal, one page for each workout. I put the date at the top, then the name of the exercise to start the line, how many reps I'm going for (if I'm going for 7 reps, I write ``(gf7)'') then next to that how many reps I achieved each set.

Write down your total time for a certain muscle group, and strive to push as much weight in as little time as possible (which is the formula for intensity!). Also give yourself a grade, A to F, on how you felt versus how you did.
I use the built in Notes app on my phone. It's simple, but effective for me. I put the date then the reps, sets, and weights for each exercise. Sometimes I'll even annotate at the end, eg good workout, too light, bad day etc.
