What is Form MV-9D?
Form MV-9D is called the Disables Persons Parking Affidavit. It is used to get a handicap parking permit in Georgia. According to the laws of Georgia, you are eligible to apply, if you have one of the following disabilities certified by a doctor:
- hearing impairment;
- lung disease;
- severe breathing difficulty;
- inability to walk 200 feet without stopping;
- severe cardiac condition;
- inability to walk without assistance;
- blindness;
- or if you use portable oxygen.
If one of these conditions applies to your situation, you may use this form.

When is Form MV-9D Due?
Once you submit Form MV-9D, it remains valid for 4 years from the date of filing. Upon the expiration of this form, it must be renewed.

What Information do I Include in Form MV-9D?
The form contains 5 sections. The first section is devoted to the information about the vehicle owner: full legal name, street address, driver’s license, county of residence, disabled person’s legal name and the relationship to the vehicle owner (self, spouse, child or ward). The second section must be filled only by the institutions where it is necessary to indicate the institution’s full legal name, vehicle make, model, year, identification and color. In section 3 you must check applicable boxes which include different types of parking permit. The fourth section must be completed by a doctor. Section 4 is a certification where the doctor confirms the disability of the individual and signs the document.
