Use the Confusion Assessment Method for the ICU (CAM-ICU) to assess the patient's level of consciousness and cognitive function. This involves four features: acute onset or fluctuating course, inattention, disorganized thinking, and altered level of consciousness.

Assess for acute onset or fluctuating course by reviewing the patient's medical records and interviewing their family or caregivers, if available. Look for evidence of rapid or sudden changes in mental status.

Evaluate for inattention by asking the patient to listen for and quickly identify specific letters (e.g., the letter "A") embedded within a series of random letters. If the patient is unable to correctly identify the target letter or makes more than two errors, they meet the criteria for inattention.

Determine if the patient has disorganized thinking by asking them to describe the current year, month, and date. If they provide an incorrect response or are unable to answer, they meet the criteria for disorganized thinking.

Lastly, assess the patient's level of consciousness by observing their responsiveness to verbal or physical stimuli. If they are unable to maintain eye contact, respond appropriately to questions or commands, or follow simple instructions, they meet the criteria for an altered level of consciousness.
