Who needs CG-1258 form?

A CG form 1258 is the US Coast Guard within the Department of Homeland Security form. Its full name is Application for Initial Issue, Exchange, or Replacement of Certificate of Documentation; Re documentation. The CG-1258 form must be completed by the owner(s) of a vessel which is to be recorded or re-documented. According to the USCG regulations, vessels used in trade operations should have a Certificate Documentation that will serve as an evidence of the vessel’s nationality.  

What is the purpose of the Form CG-1258? 

The form must be submitted in order to enable the USCG to figure out the citizenship of the owner of the vessel for which the certificate is sought and the eligibility of the vessel to be documented with the trade endorsement sought.

Is the CG-1258 Form accompanied by any other forms?

Most of the supporting documents are required if the CG-1258 Form is submitted as an application for initial issue of the Certificate of Documentation:

  • CG form 1258;

  • CG form 1261 (Builder's Certification and First Transfer of Title);

  • CG form 5397 (Application for Simplified Measurement);

  • CG form 1340 (Bill of Sale).

How to fill out the CG form 1258?

The following information is required in order to fill out the Application for Initial Issue, Exchange, or Replacement of Certificate of Documentation; Re documentation:

  • The name of the vessel;

  • The vessel’s official and other numbers;

  • Hailing port;

  • Name and contact information of managing owner;

  • Their physical and mailing address;

  • SSN or TIN;

  • Name of other owners and their SSNs or Tins;

  • Purpose of the application;

  • Endorsements for which application is made;

  • Primary service & horsepower; 

  • Citizenship of owner (an individual, government entity or corporation, etc.)

  • Certification (signature and date of completion).

The Coast Guard recently released revised CG-2692 forms, the forms for reports of marine casualties, commercial diving casualties, or outer continental shelf-related casualties. This form revision was the result of a three-year initiative to overhaul previous versions of the form, which have been largely unchanged for more than 40 years. With the new version, we’ve worked to streamline the data fields and decrease the burden of completing these forms. We will continue to strive to making reporting marine casualties easier and less time-consuming. Our long-term vision is to create the means for casualty reporting to be entirely electronic and capable of being sent directly to the appropriate Coast Guard field unit based on the geographic location provided on the form. There are many changes to this new form that you may notice at first glance. First, there are now several addendums that will ease the burden of reporting incidents with multiple injuries, deaths, or barges involved. Additionally, the form can be entirely completed electronically, including using a digital signature. Owners and operators are encouraged to work with their local Coast Guard Sectors to obtain the best and most up to date email addresses where they can file the reports. In addition, the titles and labels of the revised form have been amended to more closely correlate to the text and terms found in US statutory and regulatory language. Also, the generic, “catch-all” data fields on the previous form have also been replaced with unique fields for specific data required by statute or regulation. Lastly, notice that information requested on the forms is primarily restricted to information required by regulation, removing superfluous data fields. This form reduces the burden upon those reporting casualties by decreasing the overall number of fields required to be entered. For example, for a single vessel marine casualty, the old form required 75 data items, while the revised form only requires 40. If the incident involves one or more personnel casualties, there will only be 14 additional data items to enter, as opposed to 25 on the old form. If the incident involves one or more barges, there will only be 17 additional data items to include, as opposed to 20-35 on the old form. To help illustrate the ease of these new forms, let’s take a look at what filling out this revised form would look like. Section I covers the vessel or facility information. Block 9 is specifically set aside for additional data needed from towing vessels reporting casualties. Section II covers the reasoning for submitting the form. You may check any areas that apply to the casualty you are reporting. The sections are separated for marine casualty, commercial diving casualty, or OCS-related casualty.
