What Is the Staff Performance Appraisal Form?

Staff Performance Appraisal is an evaluation of an employee's work that involves written feedback. The feedback is provided with the help of a specific blank that includes a scale for a multi-sided assessment. This process is conducted while reviewing the employee's accomplishments in order to assign a promotion or conclude the overall performance during a certain time period.

How to Complete the Employee Performance Appraisal

The 90-day review Performance Appraisal form includes two pages. On the first page, you have to indicate the name of the person under review and the department where this person works. Next, fill out the table with a detailed scale including various assessment factors.

Dependability - indicate the degree of supervision needed for carrying out tasks properly and to what extent the person can express certain independence.
Adaptability - how flexible the person is and how fast they can grab new information related to job changes and learn new skills.
Attendance - indicate if the employer fullows the working schedule properly; how long do lunch and coffee breaks actually take.
Cooperation - the ability to co-work with culleagues and take responsibility.
Quantity of work - how fast the person carries out all tasks and how often overtime hours are needed and how much time they spend during the working day.
Quality of work - indicate the level of attention towards carried out tasks, how many errors were made and how attentive the employee was while performing any duties.
Job knowledge - the level of competence and familiarity with the assigned duties.
Reasoning - the level of consciousness towards the work. This basically means how the person makes decisions and conclusions regarding the issues arising while conducting the required work.
Potential - readiness to undertake additional responsibility while striving for career growth and taking initiative.
Interpersonal relations - ability to communicate with others on a personal level.

The second page of the Performance Appraisal must include the following information:

The general assessment of the individual that has to be chosen among several options.
Explanation of the chosen option.
The key strong points of the individual.
Achievements made by the employee.
Recommendations for further improvements.
Information on courses or training passed by the staff member.
Comments of the employee.
The date and signature of both the supervisor and the staff member put on the Staff Performance Appraisal.

Love the software but a bit of a brain surgery to start with each new document. The links, steps and path for loading and working on documents need to be highlighted better for new users. Probably makes great sense to the programmer but new users have to hunt for the next link or button to click.

Helpful Tool I had been trying to send off some release forms and was crunched for time so I signed up for PDFFiller's free trial. In the end, I wasn't able to use their software without a purchase, but it made me almost wish I had paid the money. I like that I can sign any document electronically. It definitely saves time and printing costs. It cuts out about 2 extra steps: printing and scanning. This is helpful for documents that need signatures such as release forms, etc. I didn't like how much I had to pay in order to use my e-signed pdf. I don't use the software often enough to make it worth the price. However, if I worked regularly with documents that needed to be signed this software would be the very best solution.
