trying to find the right form that was not already filled out. Also you need to add the ability to erase to the lowest membership level. You can do a couple of things such as restrict the number of new forms per month, offer a 1-5 form only fee, or only a 1 month membership fee. In the past 3 years I've used this service only twice now as I am not a business.

A child under the age of 18 is considered a minor and is unable to sign a contract unless it is for essential items. Essential items include medicines, food, and medical services. Otherwise, the minor child must have a parent or guardian consent to the contract in order for it to be legally binding.
Minors. People under the age of 18 may not be able to sign legal contracts. However, minors can be bound to contracts that someone has created for their benefit.
A child under the age of 18 is considered a minor and is unable to sign a contract unless it is for essential items. Essential items include medicines, food, and medical services. Otherwise, the minor child must have a parent or guardian consent to the contract in order for it to be legally binding.
If the child has lied about their age to buy something which is illegal, e.g. alcohol or cigarettes, then they can be held responsible, in the same way as it is illegal for an adult to purchase those substances with the intent to supply a minor to whom those substances are banned.
In the UK, a legal presumption exists that entitles anyone to enter into a contract unless an exception applies. One of those exceptions is in the case of a minor. Since 1969 the age of contractual capacity for individuals has been set at 18 and reaching the age of 18 is known as attaining 'majority'.
Without consensus there can be no agreement, and therefore no contract. For example, a well-known gym contracts with a 17-year-old and in signing the agreement, the gym states that as long as the new member remains on his current contract, he will always pay the student rate (notwithstanding inflation of course).
In the state of Alabama, 19 years old is considered the age of majority. The type of contract can affect what the legal contract age is. For example, car rentals usually have the rule that the renter needs to be a minimum of 21 years old.
Age of Majority For most states, this is 18 years of age. The age of majority may varies according to state laws, however. For example, in Alabama, the age of majority is 19 years old. The type of contract may also affect the legal contract age.
