I had one of the most celebrated times with your system technician. I had a major problem with my document processing tonight and he pulled me through. I had gone through quite a few measures trying to resolve the issue on my own, but was unsuccessful. He worked with me until we located the issue, then there was another problem on top of that one. Dear Sir/Madam, I am writing this letter to request a full refund for the clothes I ordered from your store. I had received a lot of reviews which is why I ordered a few cloth materials worth Rs. 20,000 for my painting business. I regret to inform you that I am not satisfied with the materials.
State law says physicians must return any overpayment to a patient within 30 days after determining the patient has overpaid. Because the law has no minimum balance exception, you can reasonably infer that you must refund all amounts, even tiny amounts.
My name is (Name) and over the past two / three months, I have been trying to get my refund back of admission fees. This is because I paid for the year fees but at the last minute due to very personal reasons I am not able to attend and so I started to send an application of getting my money back.
The refund shall be made as follows: (1) If the patient requests a refund, within 30 days following the request from that patient for a refund if the duplicate payment has been received, or within 30 days of receipt of the duplicate payment if the duplicate payment has not been received.
(a) A physician and surgeon and a dentist shall refund any amount that a patient has paid for services rendered that has subsequently been paid to the physician and surgeon or dentist by a third-party payor and that constitutes a duplicate payment.
Immediately send the patient a check for the overpaid amount with a note explaining the overpayment. In any case, a provider cannot just keep the overpayment – that is illegal. If an insurance carrier pays more than expected, it is important to first determine if it is truly an overpayment.
