What is a SS-5 form?

An application for a Social Security Card is used to obtain a Social Security Card, renew an existing card, or make changes to an existing card. Depending on which you are applying for, you will need to provide different documents to support your application.

To obtain a new Social Security Card, you have to provide evidence of your age, identity, and US citizenship (or work-authorized status).

To replace your existing Social Security Card, you should only prove your identity. For non-US citizens, the evidence of a work-authorized status is also obligatory.

To make changes to your Social Security Card, you have to provide evidence of your identity along with supporting documents that will act as the reason for the change.

Limits: Keep in mind that you can only obtain three Social Security Cards during a calendar year and ten in a lifetime. This rule doesn’t count for changes in your legal name and work-authorization status.

Important: The Social Security Administration does not charge fees for applying. Be aware of organizations that ask for money for their services.

Dates: It usually takes from seven to fifteen days to issue a new Social Security Card.

How to fill out a SS-5 form

The application is a one-page form that you can fill out and sign in minutes. All questions in the SS-5 form are self-explanatory. You need to add your personal information, place of birth, citizenship, and sign the form.
Below are step-by-step instructions:
