Please take note that once the user fills out the form and successfully clicks the submit button all the information that the user filled out will be sent to your Submission Inbox or Table. These submissions can be downloaded as Excel, CSV, or PDF, here is a guide on how to do that.

Position your cursor where you want to place the checkbox in the document. Get back to the "Developer" tab and click on the "Check Box Content Control" button. You will see a checkbox appear wherever your cursor is placed. To mark a checkbox, click on it and add an "X".

If the Controls task pane is not visible, click More Controls on the Insert menu, or press ALT+I, C. Under Insert controls, click Check Box.
Add a checkbox Open your Forms Page and select a form. Click on Submissions under the Data selection. In your Submission Table, click on the Add button at the right-hand side of your table. Under the Basic tab, click on Checkbox. Enter the Column name in the field provided and click the Create Column button once done.
