Answer: It depends. As long as the name on your passport and ticket match, there should be no problem. The only document you have to present when passing through customs is a valid passport.
Your passport and driver's license do not need to match. When traveling internationally, the airline or TSA will not ask for your driver's license, it is not a valid form of ID for international travel. Your passport and social security name do not need to match.
Answer: It depends. As long as the name on your passport and ticket match, there should be no problem. The only document you have to present when passing through customs is a valid passport. Since your trip isn't until April, you might consider changing the name in your driver's license.
If the name on your ticket does not match your passport, you're not flying. No airline will allow a passenger to fly internationally when the name on their passport and the name on the ticket aren't the same.
