It was helpful, but it only put the payers name on copy A, the other copies had all the information there, except the payers name, address, city,state, zip.......the main header.

For people who are not that good in technology like me (you are not to blame), it was a bit difficult to find a website where I could fill important documents to send via internet in a safe manner.
