I absolutely LOVE the convenience PDF filler provides for our patients, as well as our front desk staff. The only thing that would improve the service would be a guide that indicates to the patient that they missed a section, so they know to keep scrolling to complete all the forms.

How Much Does It Cost to Publish a Book? The cost of publishing a book varies greatly, but self-published authors can expect to spend anywhere from $100-$2500 to publish a book based on additional book production costs like editing, cover design, formatting, and more, which we cover.
On average, traditional publishers pay authors around $1.25 per book in royalties. So if your book sells for $20, you earn just over a buck.
Given the royalties of a standard contract, an author selling 20,000 books priced at $25 would earn $65,625 the first week on the “New York Times” best-seller list. This is not quite the millions' superstar authors bank, but if the book continues to sell, these earnings add up to an impressive income.
As we can see from many authors and agents the average first time author is projected to earn around $10,000 for their new book. After you pay your agent and invest in promotion, there isn't much left over.
So how much does a writer have to sell to make it? Average earnings in the UK were around £26,500 in 2012. To make this amount on a book contract for a paperback edition selling at £7.99 that pays 10% a writer would need to sell 33,166 copies a year.
The average self-published author makes around $1,000 per year according to The Guardian. That's including many authors that have multiple books and a huge list of fans. In fact, nearly a third of the authors made less than $500 a year and 90% of books sold less than 100 copies.
Not ever. You don't pay to have your book published. The reason you don't pay to have your book published is as follows: If you're a writer, then writing is your job. People get paid to do their jobs nurses get paid to nurse, ditch diggers get paid to dig ditches, and writers get paid to write.
Short answer: NO. Longer answer: You should never, full stop, pay someone to put your book out (unless you're vanity publishing a few copies for personal distribution, which is different). There are, however, occasions when you are going to need to put some money down before your book is ready.
