A high school transcript is basically a record of your academic accomplishments in high school. It lists every class you took, when you took them, and the grade you received in each class, sometimes along with additional information such as standardized test scores and any honors you received.

It includes your enrollment history, grades that you got, credits earned and attempted and average of grade-point. In case of transfer from other educational institutions the transfer credits are given. Usually a transcript covers educational history of the college or university that has issued it.
Usually at the top of the transcript, personal details are listed, such as full name, address, and birthdate, along with the total credits earned, grade point average, and class rank. Then the transcript lists all the classes taken by the student starting from the most recent, and the grade and credits earned.
The course number tells you how the course appears in the catalog. The next column is usually the title of the course. Next you should see the attempted hours/credits for that course. Next you'll see your letter grade for the course.
Transcript/Official Grade Report. A transcript is an official copy of a student's academic record detailing the courses the student has taken and each grade received. As an important document requested by schools in order to process applications, applicants must submit their transcripts in English.
