If so then you can use the form tool(s) to remove all the form fields. That'll leave you with just your PDF page content. Go to Form Edit mode (Tools — Forms — Edit), select all the fields in the list and press Delete, then return to the normal viewing mode and save the file.
What is happening in your form is that the fields that automatically get filled when you fill out one field all share the same name. This is a feature in PDF forms so that you can easily propagate information throughout a document. Whoever created your form made a mistake and named these fields the same.
